Emergency Inhaler Consent
If your child carries an asthma inhaler, please complete the form below and hand into school reception.

Medical Absence
To report an illness, please email [email protected] or call 01903 241368 and choose option 1 by 9.15am on each day of the absence. A truancy call will be sent upon any unknown student absence.
Based upon the Public Health Agency guidelines, students who have experienced vomiting and/or diarrhoea should be kept away from school for 48 hours from their last episode.
When the student is well enough, parents and carers must also send a letter in confirming the absence.
Medical or Dental Appointments
To report a medical or dental appointment, please inform the school in advance wherever possible by emailing [email protected].
Please remember to write a note in your child’s journal to confirm the absence in order for the class teacher to release your child from their lesson.
All students must sign in and out of School Reception for health and safety purposes.
Medication Request Form
This form should be completed when a child needs to bring any medication into school. Please hand deliver the form to one of our receptionists with the medication.
Prescribed GP Medication on Trips and Fixtures
Please note that it is the responsibility of the parent/carer to ensure that your child has any prescribed in-date medication e.g. Adrenaline Auto-Injector or asthma inhaler on them at all times.
